ABSTRACT

Glaucoma is still one of the leading causes of irreversible blindness worldwide, affecting an estimated 80 million people. The disease is defined by a progressive damage to the optic nerve and is often associated with an increase in intra-ocular pressure (IOP). Classical pharmacotherapy includes the different classes of IOP-lowering agents. The recently approved Rho kinase inhibitor Netarsudil is a major therapeutic advance due to its novel mechanism of action targeting the trabecular meshwork outflow pathway. However, conventional ophthalmic formulations suffer from serious limitations like low corneal bioavailability (0.1-5%), rapid precorneal clearance and dose-dependent side effects like conjunctival hyperaemia . Microemulsion based pharmaceutical delivery systems offer a promising platform to overcome these problems by enhanced permeation, prolonged ocular residence time and improved solubility of drugs. This review systematically surveys the literature on ocular delivery of Netarsudil using microemulsions, focusing on formulation approaches, characterisation parameters, optimisation strategies and therapeutic potential.
